Application for Admission

2010 RN to BSN/Articulated MNSc Baccalaureate Application

If your are not currently a licensed RN

DO NOT COMPLETE THIS APPLICATION

These instructions are provided to assist you in submitting all of the required application materials. Application deadline is Monday, March 1, 2010 by 4:00 p.m.

Make sure you have the following items available before beginning the on-line application process. Once you start completing the application you must complete it in-full. There is not a way to save the file.

P Official transcript from every college, university, or school attended since high school.

(An official transcript is required even if only one (1) course was completed at the institution.)

P A copy of your current RN licensure.

P Proof of completion of Health Assessment requirement in one of the following ways:

College credit

Continuing education credit

Will be enrolled with successful completion prior to registration.

P Ensure all prerequisite courses will be be completed with a “C” or better.

The application is 13 pages long and will take approximately 20 minutes.

You will need to print, sign, and return the final page

within 7 days before your application will be finalized.

Complete this application only if you meet the requirements below.

The undergraduate curriculum of the University of Arkansas for Medical Sciences College of Nursing leads to the Bachelor of Science in Nursing degree.

¨ Applicants must be able to show that they will have completed all of the general education prerequisites.

Please do not apply unless you are sure that all prerequisites will be completed and recorded on an official transcript by the anticipated semester of registration.

All official college transcript(s) through the summer semester must be received with the completed application and must be postmarked no later than Tuesday, September 1, 2009. This deadline will be strictly enforced due to a limited number of slots available.

Note: Final Spring 2010 transcripts are due in the office (not postmarked) of the Director of Admissions by Wednesday, May 19, 2010, by 4 PM. Official transcripts for all correspondence coursework are due at the same time also.

¨ Until all segments of the application packet have been submitted, an applicant's file is not considered complete, and it will not be reviewed. To check the status of your file, please e-mail at mmcclain@uams.edu or call Mary McClain at 501-686-8351. (Please include your legal name or SSN so your file can be identified.)

¨ Following the review by the Admission and Progression Committee, candidates will be notified of the application decision by letter from the Director of Admissions around the end of November. Applicants who submit a completed application packet by Monday, March 1, 2010, will be given priority for admission.

¨ After applicants are accepted for admission/conditional admission, they will need to send the College of Nursing a money order for $150, along with their confirmation form to hold their place in the 2010 class. When confirmed applications register for classes in January, the $150 will be applied as a credit to the tuition total. If an applicant fails to complete all prerequisite requirements for admission, does not provide official transcripts, or fails to register for nursing classes in January, the $150 will not be refunded.
